Overview
The bile duct carries bile from the liver to the intestine. Stones, narrowing (stricture), injury after previous surgery, tumors or a congenital dilatation (choledochal cyst) can block this flow and cause jaundice, fever and pain. Many duct stones are first managed endoscopically (ERCP), but surgery is needed when stones are large or multiple, when ERCP is not possible, or when the duct itself must be removed or rebuilt.

- Bile duct stones not cleared by ERCP (choledocholithotomy)
- Choledochal cyst in children or adults — complete excision is recommended because of the long-term cancer risk
- Benign stricture or bile duct injury needing reconstruction
- Selected bile duct tumors

The procedure / অপারেশন পদ্ধতি
Choledocholithotomy opens the duct to clear stones and is often combined with removal of the gallbladder. Choledochal cyst surgery removes the abnormal duct completely and restores drainage by joining a loop of intestine to the bile duct near the liver (Roux-en-Y hepaticojejunostomy). The same reconstruction is used to repair strictures and injuries.

- Hospital stay is usually about five to seven days
- A drain or tube may be kept for a short period
- Liver-function tests during follow-up to confirm good bile flow
- Long-term review, especially after choledochal cyst excision

Q. Why should a choledochal cyst be removed even without symptoms?
A choledochal cyst carries a long-term risk of infection, stones and cancer of the bile duct. Complete excision with reconstruction is the recommended treatment.
Q. Is ERCP or surgery better for bile duct stones?
Many duct stones can be removed by ERCP. Surgery is preferred when stones are large or multiple, when ERCP has failed, or when the gallbladder is being removed at the same time.
